Chino Valley Unified School District students in grades 5, 7, 9, and 11 will participate in the California Healthy Kids Survey between now and December 9th. Ayala High School will be administering the survey on Wednesday, November 17th, in English classes.
The California Department of Education requires schools to survey students about alcohol, tobacco, and other drugs, school safety, and the learning environment. The survey is anonymous and confidential. No names or any identifying information are connected to the answers.
Participation in the survey is voluntary. We hope that you will encourage your student to participate so that their opinion can be reflected in the district report and be used for program planning.
As some questions may be considered sensitive, we encourage you to view the survey in our front office or online.

To exempt a student from the survey please download the CKHS Withdrawal Form. Have your child turn the withdrawal form for this survey to their 9th/11th grade English teacher, or they may turn it in at the front office to Nichole Vicario. You may also email to email@example.com. Please turn this in no later than Tuesday, November 16th. If emailing, please put “CHKS Withdrawal” in the subject area of the email.
