CVUSD Health Center

The Chino Valley Unified School District (CVUSD) Health Center staff is happy to provide medical care to students within the CVUSD and surrounding communities! The goal of our program is to improve health outcomes by increasing student access to quality and comprehensive health care and supporting the school district's mission to improve academic achievement by working together to address absenteeism and health concerns that may pose as barriers to learning. 

Our "walk-in" vaccine clinic is every first and third Friday of the month from 7:30 a.m. to 2:00 p.m. No appointment necessary. 

The CVUSD Health Center provides the following services:

  • Immunizations for children ages 0-18 that are uninsured or have Medi-Cal or other Medi-Cal funded plans (i.e., IEHP, Molina)
  • Well-child check-ups
  • School-entry physical exams 
  • Sports Physicals
  • Diagnosis and treatment of common childhood illnesses and conditions
  • Instant on-site temporary Medi-Cal for qualifying children
  • WIC referrals for children under five years of age
  • Specialty referrals as necessary (i.e., orthopedics, cardiology, ophthalmology, endocrinology, etc.)
  • Presumptive Eligibility Program Enrollment
    • Presumptive eligibility allows children to get access to Medi-Cal services without having to wait for their application to be fully processed. Clinic staff will place qualifying children on 30 days of temporary Medi-Cal which allows children to access services such as dental and vision care.

Children's Presumptive Eligibility

The Child Health and Disability Prevention (CHDP) Gateway is being rebranded as the Children’s Presumptive Eligibility (CPE) Portal. Provider participation in Presumptive Eligibility (PE) is also being expanded. Effective for dates of service on or after July 1, 2024, most Medi-Cal and most Medi-Cal Dental fee-for-service provider types can participate in the enrollment of children into temporary coverage through PE.

Note: CPE is not a program. It is a means for providers to grant temporary, full-scope coverage to eligible applicants through an online portal.

As listed below, CPE eligibility requirements are the same as they are under the CHDP Gateway. Determinations are based on the applicant/family’s self-attestation of facts on the application.

Applicants are potentially eligible for Children’s Presumptive Eligibility if they are:

  • Under the age of 19
  • A California resident
  • Not receiving Medi-Cal
  • Within the income limits for their household size
  • Have not exceeded two PE enrollment periods in the last 12 months

Additional information will be released ahead of the July 1, 2024, implementation.
