CVUSD Health Clinic

The Chino Valley Unified School District (CVUSD) Health Center is located on the Adult School campus at 12970 Third Street, Room L2. We provide medical care to students within the CVUSD and surrounding communities.

 

The goal of our program is to improve health outcomes by increasing student access to quality and comprehensive health care and supporting the school district's mission to improve academic achievement by working together to address absenteeism and health concerns that may pose as barriers to learning. 

 

The CVUSD Health Center is currently open Monday-Friday, from 7:30 a.m. to 3:30 p.m. Patients are seen by appointment only. Please call us at (909) 628-1201 Ext. 8935. 

 

Our "walk-in" vaccine clinic is every first and third Friday of the month from 7:30 a.m. to 2:00 p.m. No appointment necessary. 

 

The CVUSD Health Center provides the following services:

  • Immunizations for children ages 0-18 that are uninsured or have Medi-Cal or other Medi-Cal funded plans (i.e., IEHP, Molina)
  • Well-child check-ups
  • School-entry physical exams 
  • Sports Physicals
  • Diagnosis and treatment of common childhood illnesses and conditions
  • Instant on-site temporary Medi-Cal for qualifying children
  • WIC referrals for children under five years of age
  • Specialty referrals as necessary (i.e., orthopedics, cardiology, ophthalmology, endocrinology, etc.)
  • Presumptive Eligibility Program Enrollment
    • Presumptive eligibility allows children to get access to Medi-Cal services without having to wait for their application to be fully processed. Clinic staff will place qualifying children on 30 days of temporary Medi-Cal which allows children to access services such as dental and vision care.

El Centro de Salud del Distrito Escolar Unificado de Chino Valley (CVUSD) está ubicado en el campus de la Escuela para Adultos en 12970 Third Street Room L2.  Brindamos atención médica a estudiantes dentro del CVUSD y las comunidades circundantes.

 

El objetivo de nuestro programa es mejorar los resultados de salud al aumentar el acceso de los estudiantes a una atencioñ médica integral y de calidad y apoyar la misión del distrito escolar de mejorar el rendimiento académico trabajando juntos para abordar el ausentismo y los problemas de salud que pueden representar barreras para el aprendizaje.

 

El Centro de Salud CVUSD está abierto actualmente de lunes a viernes, de 7:30 a.m. a 3:30 p.m. Los pacientes son vistos solo con cita previa. Por favor llámenos al (909) 628-1201 Ext. 8935.

 

Nuestra clíníca de vacunas "sin cita previa" es cada primer y tercer viernes del mes de 7:30 a.m. a 2:00 p.m. No es necesario hacer cita.

 

El Centro de Salud CVUSD ofrece los siguientes servicios:

  • Vacunas para niños de 0 a 19 años que no tiene seguro o tienen Medi-Cal u otros planes financiados por Medi-Cal (es decir, IEHP, Molina)
  • Chequeos de niños sano
  • Exámenes físicos de ingreso a la escuela
  • Exámenes físicos deportivos
  • Diagnóstico y tratamiento de enfermedades y condiciones comunes en la niñez
  • Medi-Cal temporal instantáneo para niños calificados
  • Referencias de WIC para niños menores de cinco años de edad
  • Referencias de especialidades segün sea necesario (es decir, ortopedia, cardiología, oftalmología, endocrinología, etc.) 
  • Inscripcion al Programa de Elegibilidad Presunta
    • La Elegibilidad Presunta permite a los niños tener acceso a los servicios de Medi-Cal sin tener que esperar a que su solicitud se procese por completo. El personal de la clínica colocará a los niños que califiquen en 30 días de Medi-Cal temporal, lo que permite a los niños acceder a servicios como atención dental y de la vista.

Children's Presumptive Eligibility

COMING JULY 2024: CHILDREN'S PRESUMPTIVE ELIGIBILITY

 

The Child Health and Disability Prevention (CHDP) Gateway is being rebranded as the Children’s Presumptive Eligibility (CPE) Portal. Provider participation in Presumptive Eligibility (PE) is also being expanded. Effective for dates of service on or after July 1, 2024, most Medi-Cal and most Medi-Cal Dental fee-for-service provider types can participate in the enrollment of children into temporary coverage through PE.

 

Note: CPE is not a program. It is a means for providers to grant temporary, full-scope coverage to eligible applicants through an online portal.

 

As listed below, CPE eligibility requirements are the same as they are under the CHDP Gateway. Determinations are based on the applicant/family’s self-attestation of facts on the application.

 

Applicants are potentially eligible for Children’s Presumptive Eligibility if they are:

  • Under the age of 19
  • A California resident
  • Not receiving Medi-Cal
  • Within the income limits for their household size
  • Have not exceeded two PE enrollment periods in the last 12 months

 

Additional information will be released ahead of the July 1, 2024, implementation.
