CVUSD Health Clinic

The Chino Valley Unified School District (CVUSD) Health Center is located on the Adult School campus at 12970 Third Street, Room L2. We provide medical care to students within the CVUSD and surrounding communities.

The goal of our program is to improve health outcomes by increasing student access to quality and comprehensive health care and supporting the school district's mission to improve academic achievement by working together to address absenteeism and health concerns that may pose as barriers to learning. 

The CVUSD Health Center is currently open Monday-Friday, from 7:30 a.m. to 3:00 p.m. Patients are seen by appointment only . Please call us at (909)628-1201 Ext. 8935. 

Our "walk-in" vaccine clinic is every first and third Friday of the month from 7:30 a.m. to 2:00 p.m. No appointment necessary. 

The CVUSD Health Center provides the following services:

• Immunizations for children ages 0-18 that are uninsured or have Medi-Cal or other Medi-Cal funded plans (i.e.,IEHP, Molina)

• Well-child check-ups

• School-entry physical exams 

• Sports Physicals

• Diagnosis and treatment of common childhood illnesses and conditions

• Instant on-site temporary Medi-Cal for qualifying children

• WIC referrals for children under five years of age

• Specialty referrals as necessary
