  • Absence Notice

    Your account may be eligible for tuition credit when your child will be absent from the program on the following conditions:
     
    1.  Absence Notice must be received in writing the Monday before the upcoming week of absence. Verbal notices will not be accepted;
    2.  Absence Notice must be completed in full and signed by parent. Incomplete Absence Notice requests will be denied;
    3.  Absence Notice is for a full week (Monday-Friday);
    4.  Absence Notice does not exceed the two maximum allowed: two weeks per child, per contract year (July 1-June 30);
    5.  Absence Notices will not be honored in lieu of or in combination with the Termination of Enrollment Contract.
     
    Tuition fees will not be waived for any unexpected absences, regardless of circumstances.

  • Change of Contract

    To alter an existing Enrollment Contract, a written Change of Contract is required.  A fee of $25 is required for each Change of Contract.  A $20 late fee will apply to all Change of Contracts received after the deadline.  Change of Contracts are due the Monday before the upcoming week of change. 
     
    A Change of Contract is needed to:
    1.  Add/Cancel Day(s)/Time(s):  i.e. your child attends 3 days after school and you now need 5 days after school or your child attends 3 days before and after school and you now need 3 days after school.
    2.  Change Days of Attendance:  i.e. your child attends Monday, Tuesday and Wednesday and you now need them to attend Wednesday, Thursday and Friday.
    3.  Add Unplanned/Extra Minimum Days
    4.  Transfer Programs:  Space must be available at the requested Fun Club location.  If there is no space available, parent will be placed on the wait list.  Currently enrolled families do not have priority for enrollment at other Fun Club locations.
    5.  Add a Sibling:  Additional paperwork and enrollment fees may apply.
     
    All Change of Contracts must be approved by the Office of Child Care and Development before the requested change takes effect.  Parents will be notified in writing if a Change of Contract is not approved.

  • Contract Termination

    To terminate your child's enrollment, a Termination of Enrollment Contract must be completed in writing and submitted the Monday before the upcoming week of termination; two full weeks in advance of the final attendance date.
     
    An Absence Notice will not be honored in lieu of or in combination with the Termination of Enrollment Contract.  Should you leave the program without giving the required two full weeks' written notice, your account will automatically be charged the full two weeks' tuition.  Failure to pay these fees may result in your account being sent to collections.

  • CVUSD Medication Authorization

    The Medication Authorization form must be completed by your child's physician if medication is to be administered to your child while in the care of Fun Club staff.
