Positive COVID-19 Student Attestation Form
Students isolating at home can return to campus after providing their At-Home COVID-19 antigen test result and have met isolation guidance provided by CDPH. Parents/guardians are asked to complete and return the Positive COVID-19 Student Attestation Form to their student's school office upon their student's return to school.
Opt-Out Form to Decline At-Home COVID-19 Antigen Test Kit
As an added layer of protection to mitigate an outbreak among students and staff, the California Department of Public Health (CDPH) has provided an At-Home COVID-19 test kit for each student enrolled in a California private or public school. There is no mandate or requirement that obligates students to accept a test kit.
Parents and guardians who would like to decline receiving a test kit for their student are welcome to complete an opt-out form that can be returned to their student’s school site before kits are distributed to students.
