Positive COVID-19 Student Attestation Form
Students isolating at home can return to campus after providing their At-Home COVID-19 antigen test result and have met isolation guidance provided by CDPH. Parents/guardians are asked to complete and return the Positive COVID-19 Student Attestation Form to their student's school office upon their student's return to school.
Opt-Out Form to Decline At-Home COVID-19 Antigen Test Kit
As an added layer of protection to mitigate an outbreak among students and staff, the California Department of Public Health (CDPH) has provided an At-Home COVID-19 test kit for each student enrolled in a California private or public school. There is no mandate or requirement that obligates students to accept a test kit.
Parents and guardians who would like to decline receiving a test kit for their student are welcome to complete an opt-out form that can be returned to their student’s school site before kits are distributed to students.
As prescribed by the California Department of Public Health’s (CDPH) guidance dated July 12, 2021, masks are required to be worn while indoors at K-12 schools and childcare or other youth settings, regardless of vaccination status. Exceptions are available to persons who meet specific eligibility criteria.
Parents and guardians must complete the entire application, which includes the consent for release of medical information. It may be necessary for the school nurse to contact you or your health care provider for more information.
